The HELP Center received a direct referral from Forsyth County Department of Social Services through our More Than a Job – North Carolina (MTAJ-NC) partnership. The referral packet was transmitted by the county to initiate potential enrollment into The HELP Center’s employment and training services.


The county provided a comprehensive referral package including the participant’s FNS Employment & Training (FNS E&T) Referral Form and Monthly Activity Update, Consent for Release of Information, Screening and Referral Form, Individual Employment Plan (IEP), Participant Assessment, and Client Entry Form. All documentation was received and attached to the participant’s record for review and program intake processing.


Based on the information provided by Forsyth County, the participant has been referred for enrollment consideration in the specified course associated with The HELP Center’s workforce development programming. The HELP Center will initiate enrollment outreach to the participant to introduce the program, confirm interest, and conduct the enrollment process.


Upon successful enrollment, The HELP Center will conduct a program-specific Individual Employment Plan review and update to ensure alignment with the participant’s training pathway, employment objectives, and supportive service needs. Continued case management and program participation will be documented in accordance with FNS E&T and MTAJ-NC program reporting requirements.
