Intake Case Management - Addictions Recovery Program

Intake Case Management

Date: [Date]

Time: [Two Hours]

Participant Name:

The above referenced participant visited The HELP Center to inquire about our programs and services. During the initial assessment, we determined that the Addictions Recovery Program would be a suitable option for them. The participant identified their addiction(s) as [addiction] and voluntarily enrolled in the program. They provided [type] as proof of identification and reside in Davidson County. I explained that our program is a non-intensive recovery program offering a variety of services to address issues stemming from their addiction(s). The application has been completed, uploaded to MyHELP, and all necessary documentation has been submitted.

The participant expressed a need for support in managing their addiction. We explored various coping mechanisms and treatment options available. I inquired about the participant's symptoms and provided examples to aid in identifying them.

1.       Feeling that you have to use the substance regularly

2.       Having intense urges for the substance that leave no room for other thoughts

3.       Building up a tolerance to the drug, causing you to need more

4.       Not meeting financial obligations because of the money you spend on the substance

5.       Ignoring problems that have developed as a result of using the substance

6.       Stealing to get the substance, or other behaviors started out of desperation

7.       Having withdrawal symptoms when you try to not use the substance

The participant received information about Centerstone and Mental Health Cooperative for potential intensive services. These organizations provide addiction recovery services, detox, substance use counseling, and outpatient addiction and medication-assisted treatment programs. We also highlighted how The HELP Center is a valuable asset in the recovery process. I emphasized my availability to the participant, assuring them that I am always just a call or email away. The participant's next appointment was scheduled, and they were invited to attend group meetings as well.
