NC Works - Instructions for Adding Case Notes

Log into NC Works:

  • Open the NC Works homepage.

Navigate to the Left Menu:

  • On the homepage, locate and click the left-side menu option.

Access the Assist an Individual Section:

  • From the left menu, select Services for Workforce Staff.

  • Then click on Manage Individuals

  • Click Assist an Individual.

Search for the Participant:

  • Enter the participant’s Social Security Number (SSN).

  • Click Search.


Select the Participant:

  • When the participant's name appears in the Individual List, locate the Case Note tab under the Actions column and click it.

Create a Case Note:

  • Click on Add New Case Note. A window will open.

Enter Contact Date:

  • In the Contact Date field, enter the date of the interaction or activity.


Select Program:

  • Next to Program, choose SNAP Employment and Training from the dropdown menu.


Select Application ID (App ID):

  • Next to App ID, select Active from the dropdown menu.


Choose Partner Program:

  • Next to Partner Program, select SNAP Employment and Training Services from the dropdown menu.


Enter Subject:

  • In the Subject field, enter the appropriate case note title from the Case Note Template provided in the Employee Hub.


Select Contact Type:

  • Next to Contact Type, choose the appropriate contact type from the dropdown menu (e.g., Phone, Email, In-Person).


Enter Case Note Description:

  • In the Case Note Description section, enter the details of your case note.

  • Be sure to use the Case Note Template from the Employee Hub as a guide for the structure and content.


Add a Document (Optional):

  • If you need to add a document, click on Add a Document.

  • Select the file from your computer and upload it.


Save the Case Note:

  • Once all fields are completed, click Save to finalize the case note entry.


This process ensures proper documentation of participant interactions and case management details in NC Works for SNAP E&T participants.
