Job Description
The Case Manager serves as a frontline service provider within the Case Management Department at The HELP Center, playing a pivotal role in empowering individuals and families through direct support, resource coordination, and goal-oriented service planning. Case Managers are responsible for assessing client needs, developing individualized service plans, facilitating referrals to appropriate programs and community resources, and providing ongoing guidance and support to help clients achieve self-sufficiency.
Job Responsibilities

Conduct client assessments and develop individualized service plans

Provide referrals and connections to community resources, training programs, and supportive services

Maintain accurate, timely documentation in client records and databases

Track client progress and provide ongoing support and advocacy

Collaborate with internal staff and external partners to ensure coordinated care

Participate in team meetings, trainings, and performance evaluations

Promote a welcoming and empowering environment for all participants

Qualifications

High school diploma or equivalent required

Strong interpersonal and communication skills

Ability to manage multiple tasks, meet deadlines, and maintain client confidentiality

Proficient in Microsoft Office and basic case management software

Experience in customer service, community outreach, or volunteer work is a plus

Bilingual (English/Spanish) is a plus
